Simmons was affected by the Enhanced C-Virus and became a creature, but mutated human corpses at will to absorb them as part of his body, growing massive.
No semblance of humanity remains and he has transformed into hideous creature that looks like it were made by blending together elements of many insects.
If he loses part of his body, he mutates and absorbs human corpses again to regenerate the damaged area. As a result, he's nearly invulnerable so long as there are corpses around.
It appears The Family had been constantly observing their leader's movements from the moment he was struck with the C-Virus. Some of The Family's internal communications at the time were obtained through a frantic intercept operation by FOS.

もはや人としての面影はどこにも残っておらず、数多の昆虫の要素が混ざり合って作られたかのような、醜悪な容貌へと変化を遂げている。
身体の一部が欠損すると、再び人の死骸を変異させて取り込み、損傷個所を再生させる。そのため、周囲に死骸がある限りはほぼ不死身である。
ファミリーは、自分たちの長がＣ-ウィルスを打たれてから、その動向をずっと監視していたようである。FOSの必死の傍受作戦により、そのときのファミリー内におけるやり取りの一部が入手できている。

Derek's body continues to mutate, and he enters stage three. Derek's humanity is completely gone at this point, but his survival instinct remains. He ingests multiple corpses in order to give his body the organic matter needed to assume this form. This new, hideous form resembles an amalgamation of multiple insect species. When stage-three Derek loses a limb or other body part, he can regenerate it by ingesting more corpses. He could in theory live forever as long as there are corpses for him to ingest.
The Family kept a close eye on Simmons after he was injected with the C-Virus. The FOS was able to intercept part of a message from one of The Family's members during this period.
